Q: What fields are we required to enter?

Q. What if I can’t read the writing?
A. This will be the biggest struggle while doing data entry. Our best advice is for you to do the best you can.

Q. What if someone completely different comes up?
A. Start a whole new enrollment form by clicking on the link “Enrollment Form”

Q. What if the phone number is missing on the physical Enrollment form?
A. Start a whole new enrollment form by clicking on the link “Enrollment Form”

Q. The Parent/Caregiver didn’t put the name on the form, what should I do?
A. Just put a dash “-” for the first name and a dash”-” for their last name.

Q. What if the “Area code” is missing? (Please note that a phone number only contains 7 digits i.e: 695- 8552)
A. Use the Area code number used for the visitor that did the visit.

Q. What if the Parent/Caregiver questions are left blank or missing?
A. DO NOT select anything, as we do not want to assume anything.

Q: They didn’t put a zip code, what should I do?
A: This is not a required field, so do not worry about it.

Q. They didn’t put the child’s name in the Enrollment form, what should I do?
A. Just put a dash “-” for the first name and a dash”-” for their last name.

Q, They didn’t specify the baby’s birth date/due date or it’s missing, what should I do?
A. Input January 1st, and likely year. This might take a little bit more of your time but please try to estimate the year by the “Visit Date”. This is very important to our data collection!

Q. I cannot read the Visitors name, what should I do?
A. Look on the half colored page in front of the packet you are now on. There you will find a spelled out version of the visitors name.
Q. They didn’t list the enrollment/visit date, what should I do?
A. Input January 1st, and likely year of child’s date of birth.

Q. There is nothing selected under “Received a Kit”, what should I do?
A. Go by birthdate of child.
0-12 Months “A kit”, 12-24 Month “B kit”, 24-36 Months “C kit”.

Q. They didn’t select “Visit length”, what should I do?
A. Select the 15 minute box.

Q. They didn’t select “Visit type”, what should I do?
A. Select the Office/Individual box.
